How to Register as a New Patient

For patients who have been registered with a GP in the UK previously, please bring your medical card with you, if possible.
For new babies, please bring the pink form issued from the hospital.
For EU citizens who have never registered with a GP in the UK, you will need to bring proof of residence in the UK, such as a utility bill and your passport.
For Non-EU citizens who have never registered with a GP in the UK, you will need to bring proof of residence (a utility bill, for example), your passport, and copies of any visas.
If possible, it is best to register before you need to see a doctor, because it is helpful to have the records from your previous GP. It can take as long as two months for the notes to be transferred.
We encourage every new patient to come for a special registration check up with the nurse. Do take this opportunity for a free "MOT".
If you have medical problems for which you have been receiving treatment from your own GP, please make an appointment for the doctor as soon as you can.
If you are on regular medication please bring it with you so that repeat prescriptions can be organised.

We encourage all new patients to have a registration medical check with the practice nurse. The check is completely free and takes about 20 minutes.
The purpose of the check is to find out about any existing problems for which you are receiving treatment, and also, to help you live a healthy lifestyle. The nurse will ask about

current and past illnesses and operations

illnesses that run in the family

medication and allergies

any screening tests such as cervical smears

any immunisations such as tetanus
She will check your blood pressure, weight, and height and test a sample of your urine.
You are also offered advice on

how to stop smoking
She will assess your risk of heart disease and can arrange a cholesterol check if appropriate.
